This appointment is for an MVC Mobile Unit Event. Please be aware that issues outside of MVC control may cause the event to be cancelled. Make sure you check your email or texts prior to heading to the appointment.

I understand that failing to come to my visit without canceling could affect my ability to schedule future visits.
I understand each person must have their own scheduled visit, duplicate bookings will be canceled and bookings made with an invalid email address or phone number will be canceled.
